    you can apply when you are pregnant and when the baby arrives you just give them a call and they will ask you to come in with the baby 2 weeks later. they ask for few things like discharge papers from the hospital and her birth certificate and etc. They should be able to tell you what you need to bring in. Go on there website and look for the closest office.

    forgot to add. They just recently changed few things. They really encourage moms to breastfeed so even if you decide not to they wont give you formula until the 2nd month.
